What is being measured

The primary outcome is the main question the study is designed to answer.

  1. Number of Participants With EHR Documentation of Advance Care Planning at 3 Months

EHR documentation of advance care planning at three months assessed by Natural Language Processing.

    Measured: 3 months

Secondary outcomes (5)
  1. Patient Preferences for Goals of Care

Patient preference for life prolonging care, limited care or comfort care.

Measured: Baseline (Day 1) for Control arm, and immediately after the video decision aid at Day 1 for the Video Arm

  2. Concordance Between Stated and Documented Preferences

Concordance between stated and documented preferences among participants who died during follow-up

    Measured: 6 months

  3. ACP Documentation in the EHR

Advance care planning documentation in the EHR at six months by Natural Language Processing

    Measured: at 6 months

  4. Patient Knowledge of ACP

Knowledge score of advance care planning, range 0-5, higher value is better.

Measured: Baseline (Day 1) for Control arm, and immediately after the video decision aid at Day 1 for the Video Arm

  5. Patient ACP Engagement With ACP Topics

Advance care planning engagement (ACPE) score, range 1-5, higher value is better.

Measured: Baseline (Day 1) for Control arm, and immediately after the video decision aid at Day 1 for the Video Arm
