About this study

Official title: Transvaginal Ultrasound in Bowel Endometriosis: a Retrospective Study of 10 Years on the Clinical Impact of Preoperative Diagnosis

This retrospective single-centre study evaluates how accurately preoperative ultrasound detects bowel involvement in women with deep endometriosis, using surgical findings as the reference. It also assesses whether ultrasound correctly locates and measures bowel lesions, and whether ultrasound lesion size predicts the type of bowel surgery performed (shaving, discoid excision or segmental resection). Data come from the medical records of patients operated at the Endometriosis Clinic of Hôpital Erasme, Brussels.

Read the detailed description

Deep endometriosis involving the bowel requires accurate preoperative mapping to plan surgery and counsel patients. Expert transvaginal ultrasound is recommended as a first-line imaging tool, but its real-world diagnostic performance and its value for predicting the surgical technique need further evaluation.

This is a retrospective, observational, single-centre cohort study. Patients who underwent surgery for suspected or confirmed deep endometriosis at the Endometriosis Clinic of Hôpital Erasme (HUB, Brussels) between 2018 and 2028 will be identified from the clinic database. Preoperative ultrasound reports will be compared with operative reports and histopathology.

Collected variables include ultrasound presence, location and size of bowel lesions; surgical findings and technique (shaving, discoid excision, segmental resection); and histopathological lesion size. No additional examinations or contacts with patients are planned. A waiver of informed consent has been requested from the Ethics Committee given the retrospective design.

A preliminary analysis covering patients operated between 2018 and 2025 will be performed first.

What is being measured

The primary outcome is the main question the study is designed to answer.

  1. Sensitivity of Transvaginal Ultrasound for Bowel Endometriosis Compared With Surgical Findings Confirmed by Histopathology

Percentage of patients with bowel endometriosis found at surgery (operative report) and confirmed by histopathology who were correctly identified as having bowel involvement on preoperative transvaginal ultrasound. Unit: % of patients.

Measured: From preoperative transvaginal ultrasound to surgery and histopathology report, up to 24 months (an average of 3 months)

Secondary outcomes (3)
  1. Percentage of Bowel Lesions With Concordant Location on Transvaginal Ultrasound and at Surgery

Percentage of surgically confirmed bowel lesions whose location on preoperative transvaginal ultrasound (rectum, rectosigmoid junction, sigmoid colon) matches the location described in the operative report. Unit: % of lesions.

Measured: From preoperative transvaginal ultrasound to surgery, up to 24 months (an average of 3 months)

  2. Correlation Between Bowel Lesion Diameter on Transvaginal Ultrasound and on Histopathology

Spearman's rank correlation coefficient between the largest bowel lesion diameter measured on preoperative transvaginal ultrasound (mm) and the largest lesion diameter measured on the histopathology report (mm). Unit: Spearman's rho.

Measured: From preoperative transvaginal ultrasound to histopathology report, up to 24 months (an average of 3 months)

  3. Percentage of Patients Undergoing Segmental Resection According to the Largest Axis of the Bowel Lesion on Transvaginal Ultrasound (< 3 cm vs ≥ 3 cm)

Percentage of patients treated by segmental bowel resection, the most radical surgical technique, versus conservative techniques (shaving or discoid excision), as recorded in the operative report, in the group with a bowel lesion largest axis < 3 cm on preoperative transvaginal ultrasound and in the group with a largest axis ≥ 3 cm. This outcome aims to assess whether lesion size on ultrasound can predict the need for the most radical surgery. Unit: % of patients.

Measured: From preoperative transvaginal ultrasound to surgery, up to 24 months (an average of 3 months)

Questions about this trial

What is this clinical trial studying?

An observational study of Endometriosis and Endometriosis Rectum, run by Erasme University Hospital at 1 site in Belgium.

The full registered description is in the About this study section and the official record.

Who may be eligible to take part?

The sponsor lists 4 inclusion and 2 exclusion criteria. Eligibility is decided by the study team after screening, so read the full criteria and contact a listed site.

Is this clinical trial currently recruiting?

The record says: opening soon, as of the sponsor's last update on 28 Oct 2026. Status can change between updates, so confirm with the study contact before making plans.

Where is the study taking place?

The record lists 1 site in Belgium. Whether a site is taking part right now should be confirmed with that site. See every site.

What are the possible benefits and risks?

They depend on the study and the treatment, and no study can promise a benefit. The study team explains the known risks during informed consent, before anyone joins; the record itself does not list them.

Are results available for this study?

This study is still under way. Results are not available in the source record yet.

How can a doctor refer a patient or contact the research team?

Each site lists its own contact: choose the nearest one under Trial locations. AzTrials does not enrol participants or take referrals.

What does the primary outcome measure?

Sensitivity of Transvaginal Ultrasound for Bowel Endometriosis Compared With Surgical Findings Confirmed by Histopathology (time frame: From preoperative transvaginal ultrasound to surgery and histopathology report, up to 24 months (an average of 3 months)). It is what the study was designed to measure, not a finding.

Where can I read the original study information?
Does a listing mean the treatment is approved or proven to work?

No. A listing means the study is registered. It does not show that the treatment is safe, works, or is approved by a regulator.

How often is this information updated?

AzTrials copies changed records from ClinicalTrials.gov every night and reloads the whole registry every week. The sponsor last updated this record on 28 Oct 2026.
