About this study
Official title: Study on the Application of Generative Artificial Intelligence in Perioperative Personalized Rehabilitation Practice for Older Adults With Lung Cancer
This study will evaluate whether a generative artificial intelligence (AI)-based rehabilitation support system can help older adults with lung cancer recover after lung surgery. Eligible participants aged 65 years or older who are scheduled for video-assisted thoracoscopic surgery (VATS) will be randomly assigned to receive either personalized perioperative rehabilitation guidance through the AI-based system or usual rehabilitation guidance. The system will provide guidance on exercise, nutrition, and psychological well-being before surgery and during recovery, and participants may report symptoms for follow-up guidance. The study will compare lung function and quality of life between the groups. It will also assess exercise tolerance, emergency department visits, hospital readmissions, and events related to use of the system.

What participants receive
What participants receive. In a randomised study each person is assigned to one group by chance.
AI-guided personalized rehabilitationBehavioural
Participants will use a mobile-based generative AI rehabilitation support system before and after surgery. Before surgery, the system will provide daily reminders and personalized guidance for home-based exercise and provide support related to nutrition and psychological well-being. After surgery, participants will report relevant symptoms, such as pain, shortness of breath, cough, or fever, and receive interactive rehabilitation guidance adjusted according to their reported condition. The system is supervised by the clinical team.
| Study group | Role | What they receive |
|---|---|---|
| AI-guided personalized rehabilitation | Experimental | Not described Receives: Behavioral: AI-guided personalized rehabilitation |
| Usual care rehabilitation guidance | No intervention | Not described |
What is being measured
The primary outcome is the main question the study is designed to answer.
MDASI-LC quality-of-life scores
Quality of life will be assessed using the MD Anderson Symptom Inventory for Lung Cancer (MDASI-LC), including 6 items on symptom interference with daily life and 16 items measuring symptom severity. The scores will be collected using an online questionnaire.
Measured: Before surgery; 1 week and 2 weeks after hospital discharge; and at the 1-month postoperative return visit
Forced Expiratory Volume in 1 Second (FEV1)
FEV1 will be measured using a handheld spirometer.
Measured: Before surgery, on the day of hospital discharge, and at the 1-month postoperative return visit
Forced Vital Capacity (FVC)
FVC will be measured using a handheld spirometer.
Measured: Before surgery; on the day of hospital discharge; and at the 1-month postoperative return visit.
Maximum Voluntary Ventilation (MVV)
MVV will be measured using a handheld spirometer.
Measured: Before surgery; on the day of hospital discharge; and at the 1-month postoperative return visit.
Secondary outcomes (4)
Borg Rating of Perceived Exertion During Aerobic Exercise
Perceived exertion during aerobic exercise will be assessed using the Borg Rating of Perceived Exertion (RPE) Scale. Scores range from 6 to 20, with higher scores indicating greater perceived exertion.
Measured: Before surgery and at postoperative weeks 1, 2, 3, and 4.
Emergency Department Visits
'The number of participants with an emergency department visit will be recorded.
Measured: From randomization through 24 months after surgery.
Hospital Readmissions
The number of participants readmitted to the hospital will be recorded.
Measured: From randomization through 24 months after surgery.
Adverse Events Related to the AI-Based Rehabilitation System
The number of participants experiencing adverse events related to use of the system will be recorded, including injuries associated with excessive exercise, falls, eye strain, or other harm related to system use.
Measured: From first use of the system through 24 months after surgery.
