About this study
Official title: Myocardial Energetic Phenotyping and Prognostic Value Across the Cardiac Disease Spectrum: A 31P Magnetic Resonance Spectroscopy Study
The study is designed to establish a comprehensive myocardial energetic phenotype across a broad spectrum of cardiovascular diseases using phosphorus-31 magnetic resonance spectroscopy (31P-MRS).
And the study plans to enroll 1,000 participants, comprising healthy volunteers and patients with diverse cardiac conditions. Target cohorts primarily encompass heart failure syndromes and various cardiomyopathies, with a particular focus on non-ischemic and hypertrophic phenotypes, as well as ischemic and other structural etiologies.
By integrating 31-MRS with multiparametric standard CMR, this study aims to map disease-specific metabolic signatures. The primary outcome measure is the myocardial PCr/ATP ratio, serving as a direct in vivo marker of energetic impairment. Furthermore, the study aims to explore the coupling between metabolic phenotypes and structural remodeling (tissue characterization), mechanical dysfunction, and systemic clinical biomarkers. Crucially, the study will evaluate the prognostic value of these energetic phenotypes in predicting clinical outcomes, ultimately providing novel insights into the pathophysiology and risk stratification of cardiomyopathies and heart failure.

What participants receive
What participants receive. In a randomised study each person is assigned to one group by chance.
| Study group | Role | What they receive |
|---|---|---|
| Healthy controls | Volunteers without cardiovascular disease undergoing the same CMR and 31P-MRS protocol to establish reference values. | |
| Heart failure | Patients primarily enrolled with a clinical diagnosis of heart failure, comprehensively encompassing preserved (HFpEF) and reduced ejection fraction (HFrEF) phenotypes. | |
| Non-ischemic cardiomyopathy | Patients primarily diagnosed with non-ischemic etiologies, prominently including hypertrophic cardiomyopathy (HCM), as well as amyloid, or inflammatory cardiomyopathies. | |
| Ischemic cardiomyopathy | Patients with cardiomyopathy primarily attributable to coronary artery disease. | |
| Other structural heart disease | Patients with hypertensive heart disease or valvular heart disease. | |
| Inherited cardiomyopathy | Patients with cardiomyopathy attributable to a primary gene mutation related with myocardial energy metabolism. |
What is being measured
The primary outcome is the main question the study is designed to answer.
Myocardial phosphocreatine to ATP ratio (PCr/ATP)
Myocardial PCr/ATP measured by 31P magnetic resonance spectroscopy
Measured: At baseline CMR examination
Secondary outcomes (1)
Composite Cardiovascular and Heart Failure Events
The composite clinical endpoint includes cardiovascular mortality, heart failure-related events (including hospitalization for heart failure, heart transplantation, or implantation of a mechanical circulatory support device), and documented life-threatening arrhythmias. The components of this composite outcome will be specified according to the specific cardiac disease cohort being evaluated.
Measured: Up to 3 years
