About this study
Official title: A Phase 1/2 Multicenter Clinical Study to Evaluate Safety, Tolerability, Biological Activity, and Efficacy of an In Vivo LNP-Delivered Prime Editor Complex (PM647a) for the Treatment of Alpha-1 Antitrypsin Deficiency (AATD)
The purpose of this study is to evaluate the safety, tolerability, biological activity, and initial efficacy of PM647a, an investigational Prime Editing therapy, in adults and adolescents with Alpha-1 Antitrypsin Deficiency (AATD).
AATD is caused by a mutation (change) in a gene called SERPINA1 that prevents the cells in your liver from producing a functional protein (alpha-1 anti-trypsin; AAT) that helps protect your lung tissue. PM647a is designed to precisely correct the prevalent disease-causing mutation (SERPINA1 Pi*Z (E342K)) with the goal of restoring functional AAT. This is the first study of PM647a in humans. Participants will receive a single intravenous (IV) infusion of PM647a and will be monitored closely to evaluate safety, how the body responds to treatment, changes in AAT protein levels, and whether treatment may improve signs and symptoms of AATD.
Read the detailed description
This is a Phase 1/2 open label study evaluating PM647a in adults and adolescents with Alpha-1 Antitrypsin Deficiency (AATD) who are homozygous for the SERPINA1 p.E342K pathogenic variant, also known as the PiZ allele (PiZZ genotype). Enrollment and treatment will begin with Group A, which includes participants with lung manifestations only. Group B will open subsequently and will include participants with both lung and liver manifestations or liver manifestations only. Additionally, a set of 3 to 6 participants with AATD penetrant lung disease (with or without liver disease), will be enrolled to further understand the effects of PM647a on local pharmacodynamic effects through BAL (BAL cohort).
The study will evaluate the safety of PM647a, determine an appropriate dose for future studies, and assess whether treatment restores normal synthesis and secretion of functional AAT and improves clinical measures of AATD. Participants will receive a single IV infusion of PM647a and will undergo regular safety evaluations, laboratory testing, imaging, and clinical assessments for approximately 48 weeks after treatment. Participants will then be asked to enroll in a separate long-term follow-up study to continue monitoring safety and treatment effects for an additional 14 years.

Questions about this trial
What is this clinical trial studying?
A Phase 1/2 study testing PM647a in people with Alpha-1 Antitrypsin Deficiency and Alpha-1 Antitrypsin Deficiency-associated Lung Disease, run by Prime Medicine, Inc..
The full registered description is in the About this study section and the official record.
Who may be eligible to take part?
The sponsor lists 15 inclusion and 48 exclusion criteria. Eligibility is decided by the study team after screening, so read the full criteria and contact a listed site.
Is this clinical trial currently recruiting?
The record says: opening soon, as of the sponsor's last update on 21 Oct 2026. Status can change between updates, so confirm with the study contact before making plans.
What are the possible benefits and risks?
They depend on the study and the treatment, and no study can promise a benefit. The study team explains the known risks during informed consent, before anyone joins; the record itself does not list them.
What does this combined phase mean?
Phase 1 studies test a treatment in a small group of people for the first time or in a new way, mainly to check safety and find the right dose.
Phase 2 studies give the treatment to a larger group to see whether it has the intended effect and to keep checking safety.
Not every study follows the same path, and some studies (observational studies, device studies) have no phase.
Are results available for this study?
This study is still under way. Results are not available in the source record yet.
How can a doctor refer a patient or contact the research team?
The record lists no contact. The official record names the sponsor, who can be asked directly. AzTrials does not enrol participants or take referrals.
What does the primary outcome measure?
Safety and tolerability of PM647a. Quantified by frequency and severity of treatment emergent adverse events (TEAEs) (time frame: Immediately post-infusion up to Week 48). It is what the study was designed to measure, not a finding.
Where can I read the original study information?
Does a listing mean the treatment is approved or proven to work?
No. A listing means the study is registered. It does not show that the treatment is safe, works, or is approved by a regulator.
How often is this information updated?
AzTrials copies changed records from ClinicalTrials.gov every night and reloads the whole registry every week. The sponsor last updated this record on 21 Oct 2026.
