Study population: Adults aged 50 years or older attending Severance Hospital, Gangnam Severance Hospital, or Yongin Severance Hospital and consenting to participate in a clinical cohort focused on memory disorders. Healthy young adult controls aged 19-30 years are recruited through university notice boards and online advertisements. Planned enrollment is 1,500 older adults and 50 young adult controls (25 men and 25 women). Patients receiving anti-amyloid antibody treatment are included as an observational subgroup.
Inclusion criteria Memory disorder cohort: Adults of either sex aged 50 years or older.
Healthy young adult controls: Adults of either sex aged 19-30 years. Participants must consent to participation. For patients with limited consent capacity and MMSE <15, additional consent from a family representative is obtained.
Exclusuion Criteria Memory disorder cohort: Uncontrolled acute or chronic infection, or another life-threatening systemic illness.
Healthy young adult controls:
- Any current self-reported medical, surgical, neurological, or psychiatric condition.
- Self-reported history of liver, kidney, or heart disease.
- Daily medication use.
- Alcohol consumption on the day before blood collection.
- Severe impact to the head within the past year, including a motor vehicle accident or fall.
- Loss of consciousness for any reason within the past year.

About this study
This observational study is designed as a multicenter clinical cohort to collect and bank blood specimens linked to clinical and brain imaging data for biomarker discovery and validation in memory disorders. The protocol lists Severance Hospital, Gangnam Severance Hospital, and Yongin Severance Hospital as study sites. Planned enrollment is 1,550 participants: 1,500 adults aged 50 years or older and 50 healthy young adult controls aged 19-30 years.
Participants are enrolled according to eligibility criteria and consent, irrespective of the specific medication received. Clinical information, cognitive assessments, and available brain magnetic resonance imaging (MRI) and positron emission tomography (PET) data will be collected to investigate diagnosis, disease severity, and clinical course. A total of 6 mL of research blood will be collected for banking, separated into plasma and buffy coat, and stored at -70°C. Young adult controls will provide a comparison for age-related and disease-related biomarker differences. The target follow-up duration for the memory disorder cohort is 5 years; young adult controls attend a single study visit. The overall study period is 120 months from IRB approval.
Read the detailed description
This observational clinical cohort links banked blood specimens with clinical and brain imaging data to support biomarker discovery and validation in memory disorders. The protocol specifies a multicenter design involving Severance Hospital, Gangnam Severance Hospital, and Yongin Severance Hospital. Planned enrollment is 1,550: 1,500 adults aged 50 years or older and 50 healthy controls aged 19-30 years (25 men and 25 women). Site targets are 550 at Severance Hospital, including the young controls, and 500 at each other site. The overall study period is 120 months from IRB approval.
Hospital attendees and young volunteers recruited through university notice boards and online advertisements will be enrolled by non-probability sampling after informed consent. Enrollment is independent of the specific medication received. For patients with limited consent capacity and MMSE scores below 15, additional family representative consent will be obtained under the approved consent procedures.
Prospective recruitment and specimen collection will be combined with collection of available clinical records, including records predating enrollment. Data include demographics, education, medical and family history, comorbidities, medication use, height, weight, waist circumference, laboratory findings, and cognitive and neuropsychological assessments. Measures described in the protocol include the Korean Mini-Mental State Examination, Seoul Neuropsychological Screening Battery, Clinical Dementia Rating, Global Deterioration Scale, and Short Geriatric Depression Scale. Available brain MRI and PET data will be collected and linked to clinical and biomarker findings. Additional diagnostic tests may be performed at the specialist's discretion when existing information is insufficient for diagnostic classification, with study support for the additional costs.
The target follow-up duration for the memory disorder cohort is 5 years. Available longitudinal clinical, cognitive, and imaging data will be collected; this target does not imply a uniform visit schedule or that every participant has already completed 5 years of follow-up. Additional blood sampling may be performed in treatment-related substudies according to the approved protocol and clinical course.
Young adult controls attend one visit for memory screening and blood collection. Assessments include demographics, height, weight, MMSE, Clinical Dementia Rating, Global Deterioration Scale, hematology, clinical chemistry, and APOE genotyping. Clinical chemistry includes liver and kidney function, glucose, and electrolytes to assess factors affecting biomarker interpretation.
At enrollment, a total of 6 mL of research blood will be collected for banking and separated into plasma and buffy coat aliquots for storage at -70°C in the designated freezer at the Avison Biomedical Research Center, Yonsei University College of Medicine, Seoul, South Korea. Buffy coat permits DNA extraction. Retention, secondary use, transfer, and disposal follow participant consent. Permission to use previously collected specimens will be reconfirmed and documented if consent is withdrawn.
Metabolomic and proteomic analyses will explore markers associated with diagnosis, severity, and clinical course using linked clinical and available imaging data. Candidate biomarkers may undergo further evaluation using immunoassays and biochemical or molecular methods, including cell or animal experiments where appropriate. Treatment effects and adverse effects may also be explored.
Data will be coded, with the identification key kept separately and access limited to authorized personnel. Paper records will be locked and electronic records password-protected. The principal investigator or a delegated investigator will monitor source documents, protocol adherence, and specimen management every 12 months. Procedure-related adverse events will be assessed and reported by the investigators.

What participants receive
What participants receive. In a randomised study each person is assigned to one group by chance.
| Study group | Role | What they receive |
|---|---|---|
| Memory disorder cohort | dults aged 50 years or older recruited at the three participating hospitals. The study focuses on the spectrum of memory disorders, including subjective memory impairment, mild cognitive impairment, and dementia. Blood specimens are linked to clinical data and available cognitive and neuroimaging assessments. Participants receiving anti-amyloid antibody treatment form a subgroup with repeated blood sampling before treatment, before the 5th, 7th, 14th, and 26th administrations, at week 72 or treatment termination, and when amyloid-related imaging abnormalities are suspected. Target enrollment: 1,500. | |
| Healthy young adult controls | Healthy adults aged 19-30 years recruited through university notice boards and online advertisements. Participants attend one visit for memory screening and blood collection, including hematology, clinical chemistry, and APOE genotyping. Demographic information, height, weight, Mini-Mental State Examination, Clinical Dementia Rating, and Global Deterioration Scale are collected. Target enrollment: 50 (25 men and 25 women) at Severance Hospital. |
What is being measured
The primary outcome is the main question the study is designed to answer.
Count of participants for whom blood specimens have been banked and linked to corresponding clinical data for biomarker discovery and validation. Available brain MRI and PET data will also be collected and linked; imaging availability is not required for
Count of participants for whom research blood specimens and corresponding clinical data have been collected for biomarker discovery and validation.
Measured: Cumulative collection during the 120-month study period
