About this study
Official title: A Prospective, Multicenter Phase II Clinical Study Evaluating the Efficacy and Safety of Ipilimumab N01 in Combination With Sintilimab in the Treatment of Microsatellite Instability-High (MSI-H) or Mismatch Repair-Deficient (dMMR) Gastric or Gastroesophageal Junction Adenocarcinoma
This is a prospective, multicenter phase II clinical study designed to evaluate the efficacy and safety of ipilimumab N01 in combination with sintilimab in patients with MSI-H/dMMR gastric or gastroesophageal junction adenocarcinoma.
The study comprises two cohorts. Cohort 1 plans to enroll 19 treatment-naïve patients with resectable locally advanced (cT2-4N+M0) MSI-H/dMMR gastric or gastroesophageal junction adenocarcinoma as determined by imaging (contrast-enhanced CT or contrast-enhanced MRI), who will receive 4 cycles of neoadjuvant therapy with ipilimumab N01 (1 mg/kg IV, Q6W, D1, for a total of 2 doses) in combination with sintilimab (200 mg IV, Q3W, D1) (21 days per cycle), followed by radical D2 resection, and then 13 cycles of adjuvant sintilimab monotherapy postoperatively.
Cohort 2 plans to enroll 17 patients with initially unresectable MSI-H/dMMR gastric or gastroesophageal junction adenocarcinoma, who will receive conversion therapy with sintilimab in combination with ipilimumab N01, with MDT assessment of surgical feasibility every 2 cycles; those deemed operable will undergo surgical resection followed by adjuvant sintilimab monotherapy for up to 1 year postoperatively, while those deemed inoperable will continue treatment until PD or intolerable toxicity.
- Being tested
- Ipilimumab N01, Sintilimab
- Keywords
- MSI-H/dMMR, Immunotherapy, CTLA-4 inhibitor, Gastric / Gastroesophageal Junction Adenocarcinoma
What participants receive
What participants receive. In a randomised study each person is assigned to one group by chance.
Ipilimumab N01Drug
Ipilimumab N01 will be administered intravenously at 1 mg/kg on Day 1 of each 6-week cycle (Q6W) for a total of 2 doses during the neoadjuvant or conversion treatment period.
SintilimabDrug
Sintilimab will be administered intravenously at 200 mg on Day 1 of each 3-week cycle (Q3W). In Cohort 1, sintilimab is given in combination with ipilimumab N01 for 4 neoadjuvant cycles (21 days per cycle), followed by 13 cycles of adjuvant sintilimab monotherapy after surgery. In Cohort 2, sintilimab is given in combination with ipilimumab N01 as conversion therapy until disease progression, intolerable toxicity, or until surgery; operable patients receive adjuvant sintilimab monotherapy for up to 1 year after surgery.
| Study group | Role | What they receive |
|---|---|---|
| Neoadjuvant Ipilimumab N01 Plus Sintilimab Followed by Surgery | Experimental | Participants with treatment-naïve, resectable locally advanced (cT2-4N+M0) MSI-H/dMMR gastric or gastroesophageal junction adenocarcinoma will receive neoadjuvant therapy with ipilimumab N01 (1 mg/kg IV, Q6W, D1, for a total of 2 doses) in combination with sintilimab (200 mg IV, Q3W, D1) for 4 cycles (21 days per cycle). After completion of neoadjuvant therapy, participants will undergo radical D2 resection, followed by adjuvant sintilimab monotherapy for 13 cycles, with the first adjuvant dose administered 4-8 weeks after surgery. Receives: Drug: Ipilimumab N01; Drug: Sintilimab |
| Conversion Therapy With Sintilimab Plus Ipilimumab N01 | Experimental | Participants with initially unresectable MSI-H/dMMR gastric or gastroesophageal junction adenocarcinoma will receive conversion therapy with sintilimab in combination with ipilimumab N01. Multidisciplinary team (MDT) assessment will be performed every 2 cycles to determine surgical resectability. Participants deemed operable will undergo surgical resection, followed by adjuvant sintilimab monotherapy for up to 1 year, with the first adjuvant dose administered 4-8 weeks after surgery. Participants deemed inoperable will continue study treatment until disease progression (PD) or intolerable toxicity. Receives: Drug: Ipilimumab N01; Drug: Sintilimab |
What is being measured
The primary outcome is the main question the study is designed to answer.
Pathological Complete Response (pCR) Rate (Cohort 1)
Measured: At the time of radical surgery after completion of neoadjuvant therapy (approximately 4-6 weeks after the last neoadjuvant dose)
R0 Conversion Rate (Cohort 2)
Measured: From initiation of study treatment until the date of radical surgery (assessed throughout the study treatment period, up to approximately 24 months)
