About this study

Official title: A Randomised, Double-blind, Single-Dose, Two-way Crossover Study to Compare the PK and PD, Immunogenicity, Safety and Tolerability of Filgrastim Biosimilar and Neupogen Reference Product in Healthy Volunteers

The goal of this clinical trial is to assess the pharmacokinetic similarity of Biofilgragen, a filgrastim biosimilar, compared with EU-authorized Neupogen after single subcutaneous doses in healthy adult volunteers. The main questions it aims to answer are:

  • Are the maximum observed serum concentration (Cmax) and area under the serum concentration-time curve from time zero to infinity (AUC0-∞) of filgrastim similar between Biofilgragen and EU-authorized Neupogen?
  • What is the time to reach maximum observed serum concentration (tmax) and terminal elimination half-life (t½) following administration of each product? Researchers will compare Biofilgragen and EU-authorized Neupogen to assess pharmacokinetic similarity.

Participants will:

  • Receive a single 5 μg/kg subcutaneous dose of Biofilgragen and a single 5 μg/kg subcutaneous dose of EU-Neupogen in a randomized sequence during two treatment periods.
  • Receive each study product after an overnight fast, with the two treatment periods separated by a minimum 32-day washout period.
  • Provide blood samples at predefined time points after each dose to measure serum filgrastim concentrations and determine pharmacokinetic parameters.
  • Undergo safety assessments and monitoring throughout the study, including the follow-up period.

What participants receive

What participants receive. In a randomised study each person is assigned to one group by chance.

  • BiofilgragenDrug

Biofilgragen is a filgrastim biosimilar and recombinant human granulocyte colony-stimulating factor (G-CSF). It will be administered as a single subcutaneous injection at a dose of 5 mcg/kg. Biofilgragen is intended to mirror the reference product in amino acid sequence, formulation, route of administration, and dosing schedule and to engage the same G-CSF receptor-mediated mechanisms.

  • Neupogen (filgrastim)Drug

Neupogen is the EU-authorized reference medicinal product containing filgrastim. It will be administered as a single subcutaneous injection at a dose of 5 mcg/kg under the same conditions as Biofilgragen. It serves as the active comparator for the biosimilar comparison and has well-characterized pharmacokinetic, pharmacodynamic, safety, and immunogenicity profiles.

What is being measured

The primary outcome is the main question the study is designed to answer.

  1. Maximum observed serum filgrastim concentration (Cmax)

To compare the maximum observed serum filgrastim concentration (Cmax) following administration of Biofilgragen (test product) and EU-authorized Neupogen (reference product). Serum filgrastim concentrations will be measured using a validated enzyme-linked immunosorbent assay (ELISA) from serial blood samples collected at the protocol-specified pharmacokinetic sampling time points. Cmax will be derived using non-compartmental analysis. Pharmacokinetic similarity will be assessed using the two-sided 90% confidence interval for the test-to-reference geometric mean ratio, with acceptance limits of 80% to 125%.

    Measured: Pre-dose (-0.5 h) and 0.5, 1, 1.5, 2, 3, 4, 5, 6, 8, 10, 12, 16, and 24 hours post-dose, in each treatment period.

  2. Time to reach maximum observed serum filgrastim concentration (tmax) (hours)

To determine the time to reach the maximum observed serum filgrastim concentration (tmax) following administration of Biofilgragen and EU-authorized Neupogen. The parameter will be derived from the serum concentration-time profile using non-compartmental analysis and expressed in hours. Results will be summarized descriptively.

    Measured: Pre-dose (-0.5 h) and 0.5, 1, 1.5, 2, 3, 4, 5, 6, 8, 10, 12, 16, and 24 hours post-dose, in each treatment period.

  3. Area under the serum filgrastim concentration-time curve from time zero to infinity (AUC0-inf)

To compare the area under the serum filgrastim concentration-time curve from time zero extrapolated to infinity (AUC0-inf) following administration of Biofilgragen and EU-authorized Neupogen. The parameter will be calculated from serial serum concentration measurements using non-compartmental analysis. Pharmacokinetic similarity will be assessed using the two-sided 90% confidence interval for the test-to-reference geometric mean ratio, with acceptance limits of 80% to 125%.

    Measured: Pre-dose (-0.5 h) and 0.5, 1, 1.5, 2, 3, 4, 5, 6, 8, 10, 12, 16, and 24 hours post-dose, in each treatment period; AUC extrapolated to infinity.

  4. Terminal elimination half-life of serum filgrastim (t½) (hours)

To determine the terminal elimination half-life (t1/2) of serum filgrastim following administration of Biofilgragen and EU-authorized Neupogen.

Questions about this trial

What is this clinical trial studying?

A Phase 1 study testing Biofilgragen and Neupogen (filgrastim) in people with Neutropenia (Low White Blood Cell Count), run by BioGeneric Pharma.

The full registered description is in the About this study section and the official record.

Who may be eligible to take part?

The sponsor lists 9 inclusion and 27 exclusion criteria. Eligibility is decided by the study team after screening, so read the full criteria and contact a listed site.

Is this clinical trial currently recruiting?

The record says: opening soon, as of the sponsor's last update on 20 Oct 2026. Status can change between updates, so confirm with the study contact before making plans.

What are the possible benefits and risks?

They depend on the study and the treatment, and no study can promise a benefit. The study team explains the known risks during informed consent, before anyone joins; the record itself does not list them.

What does this phase mean?

Phase 1 studies test a treatment in a small group of people for the first time or in a new way, mainly to check safety and find the right dose.

Not every study follows the same path, and some studies (observational studies, device studies) have no phase.

Are results available for this study?

This study is still under way. Results are not available in the source record yet.

How can a doctor refer a patient or contact the research team?

Contact Merna Michel (+201220229278, mirna.michael@marc-eg.org), or a site near the patient. AzTrials does not enrol participants or take referrals.

What does the primary outcome measure?

Maximum observed serum filgrastim concentration (Cmax) (time frame: Pre-dose (-0.5 h) and 0.5, 1, 1.5, 2, 3, 4, 5, 6, 8, 10, 12, 16, and 24 hours post-dose, in each treatment period.). The study lists 4 primary outcomes in all. It is what the study was designed to measure, not a finding.

Where can I read the original study information?
Does a listing mean the treatment is approved or proven to work?

No. A listing means the study is registered. It does not show that the treatment is safe, works, or is approved by a regulator.

How often is this information updated?

AzTrials copies changed records from ClinicalTrials.gov every night and reloads the whole registry every week. The sponsor last updated this record on 20 Oct 2026.
