About this study
Enhancing Perinatal Population Health through Community Care Hubs (CARE) is designed to test system level, organizational level, and workforce level strategies that may activate individual, community health worker (CHW), CHW supervisor, and system level mechanisms to improve care coordination, timely connection to prenatal and postpartum services, and downstream clinical outcomes related to maternal morbidity and mortality.

The purpose of this study is to evaluate whether multi-level enhancements to Pathways Community Hub infrastructure improve evidence based prenatal and postpartum service delivery and maternal health outcomes among pregnant and postpartum women experiencing social needs.

What participants receive
What participants receive. In a randomised study each person is assigned to one group by chance.
Integrated Data SystemsOther
Integrated Data Systems support intervention includes standardized social needs screening, improved data sharing and care coordination and health information exchange integration.
Learning CollaborativeOther
Learning Collaborative intervention includes specialized perinatal training for CHWs, peer learning and expert-led education, and trauma-informed and evidence-based care practices.
Resiliency Skill BuildingOther
Resiliency Skill Building intervention includes Community Resiliency Model training, burnout prevention and workforce well-being support and ongoing coaching for CHWs and supervisors.

The full registered description is in the About this study section and the official record.
Is this clinical trial currently recruiting?
The record says: opening soon, as of the sponsor's last update on 9 Oct 2026. Status can change between updates, so confirm with the study contact before making plans.
What are the possible benefits and risks?
They depend on the study and the treatment, and no study can promise a benefit. The study team explains the known risks during informed consent, before anyone joins; the record itself does not list them.

What does the primary outcome measure?
Timely prenatal and postpartum care connection (time frame: Prenatal care visit within first trimester AND postpartum care visit within 7-84 days after delivery). It is what the study was designed to measure, not a finding.
Where can I read the original study information?
Does a listing mean the treatment is approved or proven to work?
No. A listing means the study is registered. It does not show that the treatment is safe, works, or is approved by a regulator.
How often is this information updated?
AzTrials copies changed records from ClinicalTrials.gov every night and reloads the whole registry every week. The sponsor last updated this record on 9 Oct 2026.
