About this study

Official title: Microdystrophin (GNT0004) Gene Therapy Clinical Trial in Duchenne Muscular Dystrophy: A Phase I/II/III Study With a Dose Determination Part Followed by an Efficacy and Safety Evaluation, Quadruple Blind Placebo-controlled Part and Then by a Long-term Safety Follow up Part, in Ambulant Boys

GNT0004 is a recombinant adeno-associated viral (AAV) vector gene therapy composed of an AAV8 serotype capsid containing a sequence-optimized gene for a human microdystrophin. Duchenne muscular dystrophy (DMD) is a neuromuscular disorder caused by dystrophin gene mutations leading to absence of functional dystrophin, a key protein that prevents muscle cell damage during physiological contractions. To address the underlying cause of the disease, GNT0004 delivers an optimized dystrophin gene that is capable to produce a shortened but functional dystrophin protein in target tissues (heart and skeletal muscles) in DMD subjects. It is expected to significantly delay or slow disease progression. GNT0004 will be given as a one-time intravenous (IV) infusion to help it reach muscles throughout the body.

Read the detailed description

The study includes 3 parts. Part 1: A dose-escalation part where two dose levels were studied in five DMD boys. Based on the review of the first 12 weeks of data, the higher dose (3×10¹³ vg/kg) was considered the most efficient dose of GNT0004 with an acceptable safety profile and was selected for Part 2. This part was completed. Part 2: A confirmatory part to evaluate the efficacy and side effects of GNT0004. Participants are randomly assigned to receive either GNT0004 or placebo in a blinded manner. This part lasts 18 months. After 18 months, participants initially receiving placebo will receive GNT0004, and those initially receiving GNT0004 will receive placebo. Part 3: A long-term follow-up part in which all participants who received GNT0004 in Parts 1 and 2 will be followed for a total of 5 years from dosing to monitor potential late-onset side effects and assess the duration of the treatment's beneficial effects. The five participants from Part 1 are currently being followed in Part 3.

What is being measured

The primary outcome is the main question the study is designed to answer.

  1. PART 1 - Safety Outcomes: Incidence of adverse events (AEs), serious adverse events (SAEs), adverse events of special interest (AESIs), adverse drug reactions (ADRs) and clinically significant laboratory and clinical examination findings

Measured: From the Screening Visit through the end of Part 1 (Week 12)

  2. PART 1 - Immunogenicity Outcomes: Antibodies to AAV8, antibodies to recombinant dystrophin (hMD1) and of cellular immune biomarkers (ELISPOT tests)

Measured: From the Screening Visit through the end of Part 1 (Week 12)

  3. PART 1: Vector shedding quantification in blood, urine, saliva, feces (DNA level)

Measured: Once a week through Week 12

  4. PART 1: Percentage change from baseline on Creatine kinase (CK) in blood over time

Measured: From the Screening Visit through the end of Part 1 (Week 12)

  5. PART 1: Quantification of Microdystrophin positive myofibers level from muscle biopsy at Week 8

    Measured: Week 8

  6. PART 1: Quantification of Microdystrophin protein expression from muscle biopsy at Week 8

  7. PART 1: Quantification of vector copy number (VCN) over time

Measured: From the Screening Visit through the end of Part 1 (Week 12)

  8. PART 2: Change from baseline in the North Star Ambulatory Assessment (NSAA) total score at Month 18

The NSAA is a validated unidimensional functional scale for ambulant male patients with DMD. An age-appropriate modified NSAA validated for young ambulant male patients from the age of 3 years with DMD is used in this study.

This scale consists of 17 functional assessments, ranging from standing to running and includes several items assessing abilities that are necessary to remain functionally ambulant in the context of DMD. Each item can be scored on a 3 point-scale from 2 ("achieves goal without any assistance") to 0 ("unable to achieve independently"). A total score (maximum "34", minimum "0") is obtained by summing all the individual items, with higher scores reflecting better functional performance.

Questions about this trial

What is this clinical trial studying?

A Phase 2/3 study testing GNT0004 (1x10e13 vg/kg) and GNT0004 (3x10e13 vg/kg) in people with Duchene Muscular Dystrophy, run by Genethon at 14 sites in 4 countries.

The full registered description is in the About this study section and the official record.

Who may be eligible to take part?

The sponsor lists 13 inclusion and 16 exclusion criteria. Eligibility is decided by the study team after screening, so read the full criteria and contact a listed site.

Is this clinical trial currently recruiting?

The record says: looking for participants now, as of the sponsor's last update on 13 Oct 2026. Status can change between updates, so confirm with the study contact before making plans.

Where is the study taking place?

The record lists 14 sites in France, Belgium, Spain, United Kingdom. Whether a site is taking part right now should be confirmed with that site. See every site.

What are the possible benefits and risks?

They depend on the study and the treatment, and no study can promise a benefit. The study team explains the known risks during informed consent, before anyone joins; the record itself does not list them.

What does this combined phase mean?

Phase 2 studies give the treatment to a larger group to see whether it has the intended effect and to keep checking safety.

Phase 3 studies compare the treatment with the current standard treatment or a placebo in a large group, to confirm how well it works and watch for side effects.

Not every study follows the same path, and some studies (observational studies, device studies) have no phase.

Are results available for this study?

This study is still under way. Results are not available in the source record yet.

How can a doctor refer a patient or contact the research team?

Contact Medical Lead (+33 01 69 47 25 38, contact@genethon.fr), or a site near the patient. AzTrials does not enrol participants or take referrals.

What does the primary outcome measure?

PART 1 - Safety Outcomes: Incidence of adverse events (AEs), serious adverse events (SAEs), adverse events of special interest (AESIs), adverse drug reactions (ADRs) and clinically significant laboratory and clinical examination findings (time frame: From the Screening Visit through the end of Part 1 (Week 12)). The study lists 9 primary outcomes in all. It is what the study was designed to measure, not a finding.

Where can I read the original study information?
Does a listing mean the treatment is approved or proven to work?

No. A listing means the study is registered. It does not show that the treatment is safe, works, or is approved by a regulator.

How often is this information updated?

AzTrials copies changed records from ClinicalTrials.gov every night and reloads the whole registry every week. The sponsor last updated this record on 13 Oct 2026.
