Study population: Women aged 18 years or older admitted to hospital with a first episode of heart failure at 24 participating Spanish centers, all of which have a dedicated heart failure unit. Recruitment is consecutive and all-comers: every woman meeting the criteria during the recruitment period is offered participation, and discretionary selection by investigators is not permitted. Each center declares a fixed screening frame in advance and keeps a screening log of women screened, women confirmed eligible, women enrolled, and eligible women not enrolled with the reason. Concurrent participation in an interventional trial is not an exclusion criterion but is recorded.
Who may be able to join
- Women aged 18 years or older.
- Hospital admission with a first episode of heart failure confirmed by the treating team according to current clinical practice guidelines, irrespective of left ventricular ejection fraction.
- Written informed consent given by the participant after verbal and written information.
Who cannot join
- Total functional dependence, defined as a Barthel index score of 20 or less (0-100 version).
- Moderate-to-severe cognitive impairment, defined as 5 or more errors on the Pfeiffer questionnaire, adjusted for educational attainment.
- Severe frailty, defined as a Clinical Frailty Scale score of 6 or more.

About this study
Official title: HER-HF: Heart Failure in Women. Epidemiological and Prognostic Insights. A National, Prospective, Multicenter Observational Cohort Study of Women Hospitalized for a First Episode of Heart Failure
Heart failure (HF) is a leading cause of hospital admission in older adults and women account for more than half of these admissions, yet they remain under-represented in prospective HF cohorts. Risk factors specific to women, such as complications of pregnancy and the timing of menopause, are seldom recorded in HF registries, and treatment is usually described by drug class rather than by the dose actually achieved.
HER-HF is a national, prospective, multicenter observational cohort study that enrolls women aged 18 years or older admitted to hospital for a first episode of HF, irrespective of left ventricular ejection fraction, at 24 Spanish hospitals with a dedicated heart failure unit. Recruitment is consecutive and all-comers. Participants are enrolled at discharge from the index admission or at the first heart failure unit visit within 15 days, and all baseline data refer to the index hospitalization. Participants are followed for 12 months, with visits at 3, 6 and 12 months.
The primary objective is descriptive: to characterize the epidemiological, clinical, diagnostic, therapeutic and prognostic profile of women with de novo HF, from the index admission through 12 months. Secondary objectives are: (1) to evaluate the association of sex-specific risk factors with the clinical profile, the type of HF (ejection fraction phenotype and etiology), the management received and 12-month prognosis, adjusted for traditional risk factors; (2) to examine the association of individual social determinants (household income, number of people in the household and educational attainment) with the type of HF, age at onset, management and outcome; and (3) to compare the cohort with a contemporary cohort of men with newly diagnosed HF (TIDY-HF registry) to identify sex-specific differences. The principal prognostic endpoint is the composite of all-cause death or HF decompensation (hospitalization for HF, or an unplanned emergency department or day-hospital visit for HF) within 12 months.
Read the detailed description
Data are collected in a centralized, encrypted electronic case report form. Baseline variables include demographics, cardiovascular risk factors and comorbidity, previous cardiological history and prior medication; sex-specific risk factors recorded in the clinical history of the index admission (number of pregnancies and deliveries; preeclampsia, eclampsia or HELLP syndrome; gestational hypertension and gestational diabetes; preterm delivery; intrauterine growth restriction or small-for-gestational-age newborn; peripartum cardiomyopathy; recurrent pregnancy loss; menopausal status and age at menopause; hormone replacement therapy; polycystic ovary syndrome; and endometriosis); previous cardiotoxic cancer therapy (chemotherapy or radiotherapy), recorded with the oncological history as a female-predominant risk factor; individual social determinants (net monthly household income, number of people in the household and educational attainment); clinical features of the index episode; laboratory, electrocardiographic and echocardiographic data; cardiac magnetic resonance, coronary angiography and other investigations when performed; treatment at discharge by drug class and dose band, with adverse effects and whether these led to dose reduction or withdrawal; devices and cardiac interventions; and the Barthel index, the Pfeiffer questionnaire, the five-item Geriatric Depression Scale and the 12-item Kansas City Cardiomyopathy Questionnaire.
At each follow-up visit (3, 6 and 12 months) the following are recorded: functional class, physical examination for residual congestion, weight and blood pressure, laboratory tests and electrocardiogram, current treatment with doses, tolerability and adverse effects, emergency department attendances and day-hospital treatments for heart failure, hospitalizations for heart failure, for other cardiovascular causes and for non-cardiovascular causes, referral to cardiac rehabilitation and referral for advanced therapies. Echocardiography is performed at the index admission, once during follow-up at the 3- or 6-month visit, and again by the 12-month visit. Vital status and cause of death are recorded throughout; clinical events are ascertained by the site investigator from the clinical and regional electronic health records, without central adjudication. Functional and health-status scales are repeated at 12 months.
To document and control selection bias, each site declares a fixed screening frame in advance and maintains a screening log recording the number of women screened, the number confirmed eligible, the number enrolled, and the number of eligible women not enrolled with the reason. Participation rates will be reported by site.
The sample size was calculated for descriptive precision: with the most conservative proportion (p = 0.50), 95% confidence and an absolute precision of 5%, 385 evaluable women are required; allowing for 10% loss to follow-up over 12 months, 430 women will be recruited. Results will be presented overall and by ejection fraction phenotype according to the 2026 European Society of Cardiology definitions (reduced, below 50%; preserved, 50% or higher; also reporting 40% or lower and 41-49% separately) and by age (under 75 or 75 years and older). Time to the composite endpoint will be analyzed with Kaplan-Meier curves and Cox models with standard errors clustered by center; all secondary analyses are exploratory. The comparison with the men of the TIDY-HF registry will include all women for clinical profile and investigations, and women with an ejection fraction of 40% or lower, the population enrolled in TIDY-HF, for the class and dose of foundational medical therapy, with adjustment for age.

What participants receive
What participants receive. In a randomised study each person is assigned to one group by chance.
| Study group | Role | What they receive |
|---|---|---|
| Women with a first hospitalization for heart failure | Women aged 18 years or older admitted to hospital with a first episode of heart failure confirmed by the treating team according to current clinical practice guidelines, irrespective of left ventricular ejection fraction, and with no previous diagnosis of heart failure. Participants are enrolled consecutively (all-comers) at discharge from the index admission or at the first heart failure unit visit within 15 days of discharge, and are followed for 12 months with visits at 3, 6 and 12 months. No intervention is assigned by the protocol; all diagnostic and therapeutic decisions are made by the treating clinician as part of routine care. |
What is being measured
The primary outcome is the main question the study is designed to answer.
Composite of all-cause death or heart failure decompensation
Time from enrollment to the earlier of death from any cause or heart failure decompensation, defined as hospitalization with heart failure as the principal reason for admission, or an unplanned emergency department or day-hospital visit for worsening heart failure. Events are ascertained by the site investigator from the clinical and regional electronic health records at each scheduled visit; there is no central adjudication.
Measured: 12 months from enrollment
Secondary outcomes (12)
All-cause mortality
Proportion of participants who die from any cause during follow-up, with date and cause of death recorded and classified as cardiovascular or non-cardiovascular.
Measured: 12 months from enrollment
Rehospitalization for heart failure
Number of participants with at least one hospital admission with heart failure as the principal reason for admission, and the total number of such admissions.
Measured: 12 months from enrollment
Prevalence of sex-specific and female-predominant risk factors
Proportion of participants with each recorded sex-specific risk factor: number of pregnancies and deliveries, preeclampsia, eclampsia or HELLP syndrome, gestational hypertension, gestational diabetes, preterm delivery before 37 weeks, intrauterine growth restriction or small-for-gestational-age newborn, peripartum cardiomyopathy, recurrent pregnancy loss, premature menopause before 40 years, hormone replacement therapy, polycystic ovary syndrome and endometriosis; and proportion with previous cardiotoxic cancer therapy (chemotherapy or radiotherapy), recorded as a female-predominant risk factor.
Measured: Baseline (index hospitalization)
Distribution of left ventricular ejection fraction phenotype
Proportion of participants in each left ventricular ejection fraction category measured by transthoracic echocardiography during the index hospitalization: reduced (below 50%) or preserved (50% or higher) according to the 2026 European Society of Cardiology definitions, also reporting 40% or lower and 41-49% separately.
Measured: Baseline (index hospitalization)
Use and achieved dose of disease-modifying heart failure therapy
Proportion of participants receiving each class of disease-modifying therapy (angiotensin-converting enzyme inhibitors, angiotensin receptor blockers, angiotensin receptor-neprilysin inhibitors, beta blockers, mineralocorticoid receptor antagonists, finerenone, sodium-glucose cotransporter 2 inhibitors, vericiguat, ivabradine, diuretics and potassium binders) and the dose band achieved for each.
Measured: Discharge from the index hospitalization and 3, 6 and 12 months
Adverse effects limiting disease-modifying therapy
Proportion of participants with hypotension, bradycardia, conduction disturbance, worsening renal function, hyperkalemia or other adverse effects attributed to disease-modifying therapy, and whether these led to dose reduction or withdrawal of the drug.
Measured: 12 months from enrollment
Change in the Kansas City Cardiomyopathy Questionnaire (KCCQ-12) overall summary score
Change from baseline in the overall summary score of the 12-item Kansas City Cardiomyopathy Questionnaire. The score ranges from 0 to 100, with higher scores indicating better health status.
Measured: Baseline and 12 months
Functional and cognitive status scales
Barthel index (0 to 100, higher scores indicate greater independence), Pfeiffer Short Portable Mental Status Questionnaire (0 to 10 errors, higher scores indicate greater cognitive impairment) and the five-item Geriatric Depression Scale (0 to 5, higher scores indicate more depressive symptoms).
Measured: Baseline and 12 months
Emergency department attendances and day-hospital treatments for heart failure
Number of participants attending the emergency department or receiving intravenous diuretic treatment in a day-hospital setting for worsening heart failure, and the number of such episodes.
Measured: 12 months from enrollment
Referral to cardiac rehabilitation and to advanced heart failure therapies
Proportion of participants referred to a cardiac rehabilitation program and proportion referred for evaluation for heart transplantation or ventricular assist device.
Measured: 12 months from enrollment
Hospitalization for cardiovascular causes other than heart failure and for non-cardiovascular causes
Number of participants admitted to hospital for a cardiovascular cause other than heart failure, and number admitted for a non-cardiovascular cause, with the number of such admissions.
Measured: 12 months from enrollment
Individual social determinants
Distribution of net monthly household income (euros), number of people in the household (including the participant) and educational attainment, recorded in six categories: unable to read or write or incomplete primary education; complete primary education; lower secondary education; upper secondary education or vocational training; university education; unknown or declined to answer. These variables are analyzed as exposures against the type of heart failure, age at onset, management and outcome.
