About this study

Official title: Minimizing Surgical Intervention in Tubal Ectopic Pregnancy: A Randomized Comparison Between Sequential and Combined Letrozole and Single-Dose Methotrexate Regimens.

This study evaluates two medical strategies designed to minimize surgical intervention and reduce methotrexate (MTX) exposure in women with undisturbed tubal ectopic pregnancy (TEP). Participants with undisturbed TEP and pre-treatment serum $\beta$-hCG levels $<3500\text{ IU/L}$ are randomized to receive either a sequential regimen (initial letrozole monotherapy, reserving MTX only for treatment non-responders) or a combined regimen (simultaneous letrozole and single-dose MTX from day one). The study aims to compare the overall success rate of resolving TEP without emergency surgery, as well as the ability of the sequential approach to avoid MTX administration altogether.

Being tested
Letrozole, Methotrexate
Medical subject headings
Pregnancy, Ectopic

What participants receive

What participants receive. In a randomised study each person is assigned to one group by chance.

  • LetrozoleDrug

Oral Letrozole 5 mg/day (2 times 2.5{ mg} tablets daily) for 4 days. If serum beta-hCG drops by >10% on Day 4, Letrozole is continued.

  • MethotrexateDrug

A single intramuscular injection of Methotrexate (50mg/m) is administered as rescue therapy, If beta-hCG drop is <10 on Day 4,

Study groups in NCT07842796
Study groupRoleWhat they receive
Sequential ProtocolActive comparatorParticipants in this arm initially receive oral Letrozole at a dose of 5 mg daily (administered as two 2.5 mg film-coated tablets once daily) for 4 consecutive days. On Day 4, serum beta-hCG levels are evaluated to specify if rescue needed we will give a single intramuscular rescue injection of Methotrexate (50mg/m) while continuing Letrozole for an additional 7 days.
Receives: Drug: Letrozole; Drug: Methotrexate
Combined ProtocolActive comparatorParticipants in this arm receive simultaneous dual drug therapy starting on Day 1
Receives: Drug: Letrozole; Drug: Methotrexate

What is being measured

The primary outcome is the main question the study is designed to answer.

  1. Letrozole Standalone Success Rate

Resolution of tubal ectopic pregnancy (confirmed by TVU and beta-hCG < 15 IU/L) achieved via Letrozole monotherapy without requiring Methotrexate or surgical intervention

Measured: 19 days post-initiation oftreatment treatment.

Secondary outcomes (1)
  1. Avoidance of Surgery

: Resolution of ectopic pregnancy (beta-hCG < 15 IU/L) using medical management alone, avoiding conversion to emergency surgery (salpingectomy/salpingotomy).

Measured: 19 days post-initiation oftreatment treatment.

Questions about this trial

What is this clinical trial studying?

A Phase 2/3 study testing Letrozole and Methotrexate in people with Ectopic Pregnancy, run by Benha University at 1 site in Egypt.

The full registered description is in the About this study section and the official record.

Who may be eligible to take part?

The sponsor lists 5 inclusion and 5 exclusion criteria. Eligibility is decided by the study team after screening, so read the full criteria and contact a listed site.

Is this clinical trial currently recruiting?

The record says: under way, not looking for participants, as of the sponsor's last update on 25 Sep 2026. Status can change between updates, so confirm with the study contact before making plans.

Where is the study taking place?

The record lists 1 site in Egypt. Whether a site is taking part right now should be confirmed with that site. See every site.

What are the possible benefits and risks?

They depend on the study and the treatment, and no study can promise a benefit. The study team explains the known risks during informed consent, before anyone joins; the record itself does not list them.

What does this combined phase mean?

Phase 2 studies give the treatment to a larger group to see whether it has the intended effect and to keep checking safety.

Phase 3 studies compare the treatment with the current standard treatment or a placebo in a large group, to confirm how well it works and watch for side effects.

Not every study follows the same path, and some studies (observational studies, device studies) have no phase.

Are results available for this study?

This study is still under way. Results are not available in the source record yet.

How can a doctor refer a patient or contact the research team?

The record lists no contact. The official record names the sponsor, who can be asked directly. AzTrials does not enrol participants or take referrals.

What does the primary outcome measure?

Letrozole Standalone Success Rate (time frame: 19 days post-initiation oftreatment treatment.). It is what the study was designed to measure, not a finding.

Where can I read the original study information?
Does a listing mean the treatment is approved or proven to work?

No. A listing means the study is registered. It does not show that the treatment is safe, works, or is approved by a regulator.

How often is this information updated?

AzTrials copies changed records from ClinicalTrials.gov every night and reloads the whole registry every week. The sponsor last updated this record on 25 Sep 2026.
